To assess the outcome of retinal detachment (RD) associated with axial length changes in eyes subjected to blunt trauma. Twenty-eight patients with axial length measurement at the beginning of an RD, and 22 patients in whom RD developed over a follow-up period of 6 months to 5 years were examined for the presence of traumatic axial elongation. A total of 43 eyes with pre- and posttrauma axial length measurements were available for analysis. The mean preoperative axial length was 25.9 +/- 2.5 mm and that at the end of follow-up, 24.2 +/- 2.6 mm. There was no significant difference between the preoperative and the final axial length (P=0.74). The median increase in axial length was 0.7 mm (range, -1.6 to 3.4 mm) in the 23 eyes with RD. There was no significant correlation between the degree of anterior chamber depth change and the axial length (P=0.38). The amount of trauma was not correlated with the degree of axial length increase (P=0.19). When assessed as a binary variable, the degree of trauma did not appear to be an indicator for the degree of axial length change. At the end of the follow-up, 21 eyes had persistent RD, of which 14 (67%) were associated with macular holes.
